Do not move MedicationRequest.dosageInstruction underneath dose

      Although we added the BackboneElement to make it clear that these three elements were semantically tied together, we do agree with the argument that it is not backwards compatible, so we will remove the 'dose' BackboneElement and make renderedDosageInstruction, effectiveDosePeriod, and dosageInstruction all first-level elements.

    Description

      For MedicationRequest.dose, we disagree with the placement of dosageInstruction underneath the BackboneElement of dose. This change is not backwards compatible and it's unnecessary.  If the additional two elements are needed (renderedDosageInstruction and effectiveDosePeriod), then they can be added as first level elements.
