Need to add a new page on deriving forms that talks about expectation for use of derived from, the fact you can constrain but not add, and the use of optionalDisplay. Also need to explain what happens with the canonical URL. Will create an example showing a parent and derived Questionnaire. Will remove the row for optionalDisplay in the examples page as it's not something that affects rendering directly. Will also move the extension from the 'rendering' to the base Questionnaire profile. Add cross-references between the new page and the modular forms. Will also add a link from the optionalDisplay extension to the new 'derivation' page.
