add BSNR and XX to ValueSet/identifier-type

      All three requested codes are sub-types of PRN - Provider Number.  The intent of this value set is to include just the top-level codes from the underlying code system.  To send the requested codes, an instance would include two repetitions of Coding, one with 'PRN' and one with the more specific code.

      We also noticed that these codes appear to correspond to an identifier system/namespace instead of an identifier type.  If that is the case, a proper identifier system should be used instead of sending these codes.
