Remove DEQM Practitioner profile and replace all references to the profile (e.g. $care-gaps operation parameter, examples) with the QI-Core STU5 Practitioner profile. Add guidance for users upgrading to newest DEQM version about migrating to new profile, as well as to the DEQM Organization profile (which was also removed, use QICore Organization now) Specifically, add a section to "Default Profiles": https://hl7.org/fhir/us/davinci-deqm/guidance.html#default-profiles-used-in-the-evaluation-of-a-measure To discuss the use of QICore Practitioner, PractitionerRole, and Organization profiles to model reporters and participating practitioners and organizations. Add a note to that section to provide an update mapping for the DEQM-level profiles that were removed (DEQM Organization, DEQM Practitioner, DEQM Coverage) In addition, update each of the Guidance pages (Data Exchange, Individual Reporting, Summary Reporting, Gaps in Care) to appropriately reflect use of QICore profiles for organization, practitioner, and coverage.
