A new Gender Identity extension has just been published (will be published before 2022-05-14) in the US Core IG - update from using http://hl7.org/fhir/R4/extension-patient-genderidentity.html to using http://build.fhir.org/ig/HL7/US-Core/StructureDefinition-us-core-genderIdentity.html for Gender Identity to align with the latest SOGI work.

The us core extension is based on the original extension, it just adds further constraints.
