Diagrams in 2.4.5.1.1 and 2.4.5.2.1 should include subscription confirmation exchange, or narrative in 2.4.5.1 and 2.4.5.2 should indicate that the subscription denial occurs before subscription confirmation.

Clarify whether the denial exchange happens with the subscriber or to the subscribed system. I think the current text intends the subscribed system, but says the subscriber. It also makes some sense to send the denal to the subscriber since it might want an opportunity to change what it has subscribed the other system to.
