      Diagram is oversimplifies steps- it's unclear whether History & Physical is trying to represent a behavior (like above) or if its supposed to represent a physical object being exchanged. Call it out as a behavior clearly similar to above (e.g., Sent the patient's latest H&P) or if the latter, better clarify what objects are being sent from the list in 2.6.4 (is H&P encopassing the attachment, supporting data, metadata for association resources?).
