    Description

      as noted-supply an example of the type of documents (named) and example (list) of data elements that would be attached to a provider message sent to a payer.one example above the arrow and the other below the arrow. The term "re-association" in the diagram is confusing as the attachment supposedly did not exist in the record befor-association would seem to be more appropriate

      Proposed Wording:

      Would change to a more generic representation to represent any attachment that would be submitted-e.g. documents such as: History and Physical,, study report (pathology, radiology, etc.), any missing information requested by the payer-provider details, and an exampole of data elements-lab results with reference ranges, other examples of data a payer usually requests to supplement a claim. re-association, change to association
