Some of the content in DTR (e.g. storing DocumentReference on the payer side) is intended as interim support while not EHRs support storing QuestionnaireResponse with the relevant extensions.  We should make this portion of the specification clearly distinct so that we can more easily deprecate it going forward and so that EHRs that implement the desired target state can easily ignore the functionality.  (Payers will always need to support the interim process so long as any of their members interact with EHRs that only support transitional functionality.)
