We need two coverage profiles based on the CRD coverage profiles, that add in the endpoint URL.  (Alternatively, we could just update the CRD profiles to add the extension there.)

The description of the profiles needs to indicate why they exist - specifically to indicate the type of plan as well as to potentially communicate to the DTR system what endpoint to use to communicate with the payer associated with the coverage.  ("used to identify the payer" isn't sufficient.)
