"PDF containing the QuestionnaireResponse"

When we store the results as a DocumentReference on the EHR, the QR will be a PDF, but when we store a DocumentReference on the payer, the QuestionnaireResponse will be a full-blown FHIR instance.  It has to be in order to allow continued editing or to allow the payer to retrieve it and process it once they receive a reference to it in a prior auth or claim.

(Should we set an expectation whether it'll be JSON or XML, or should we allow either?)
