"For cases where the DTR process is being launched outside the context of Coverage Requirements Discovery workflow, please see Section 4.4.8."
Actually, this section hasn't provided any specific guidance about launch from CRD. The piece that's relevant for CRD launch is "What must the context token passed from hook to app be?" That needs to be standardized because we want SMART apps to be substitutable.
Also, all launch information - whether from CRD or stand-alone (and whether 'initial' stand-alone, or launching from work-in-progress) should all be covered in one section. There is no reason to have separate sections.
