We should update to reflect the expectations of the newest SMART app launch spec.  Specifically, we need to ensure our new parameters aren't confused for generic resource-specific launch parameters (which are handled as 'mandatory' and will cause the EHR to prompt the user for them if they're not already in active context).  We also need to define whether we expect the results will be passed in via the fhirContext element or something else.
