Change the example bindings for value sets to Preferred for the following QICoreCarePlan and QICoreGoal elements:

QICoreCarePlan
- CarePlan.category uses http://hl7.org/fhir/ValueSet/care-plan-category which indicates Include codes from http://snomed.info/sct where concept is-a 734163000 (Care plan) - change to "Preferred" considering some may choose to use ICD-10 instead of SNOMED
- CarePlan.category:AssessPlan uses http://hl7.org/fhir/ValueSet/care-plan-category which indicates Include codes from http://snomed.info/sct where concept is-a 734163000 (Care plan) - change to "Preferred" considering some may choose to use ICD-10 instead of SNOMED
