Use Claim.billablePeriod instead of creating a new extension

      This extension can be used to connect multiple GFEs together and to a period of care. It is not an id or date for a specific claim.

      Will add clarity and guidance for using the gfeServiceLinkingInfo extension to enable the ability to have the same identifier on multiple GFEs that are part of one period of care's services. This would be needed for some of the workflow options that businesses may decide to use. 

      Will add documentation that the billablePeriod is per claim, whereas planned period of service can go across multiple claims.
