simplify and modify menu for easier and more intuitive navigation:

1) Remove the Table of Contents - its ugly, it clutters the bar, is already in the breadcrumbs, and should not be needed for a well constructed web site.

2) Remove Base Specs - Plenty of links in the documentation, header, yellow box and footer

3) Remove history - is already linked in yellow publisher box and footer of every page.  Instead add a link to the introduction.

4) Reader expects to land on page when clicking on the menu links so only links to pages (not sections - except for FHIR Artifacts )  in the dropdown. 

5) create new menu Item Background and move content from:

• Technical Background and Underlying Technologies page. 
•  Relation to Other IGs  section
• Overview and Use Case page 
• 4.1.4  System Actors
• section 4.2.1 Summary 

   to this new page 

6) Change PCT Specification to Specification - content = formal specification

7) M_ove section 2.2 acronym table to the _Home page

8) Move Terminology Licensure page to Artifacts dropdown  item

9) Move credits to  bottom of Home page

10) create new menu item Downloads and move section 1.2 to a new Downloads page
