Bind example value sets where ever possible instead of creating complicated element definitions

    Description

      MeasureReport.group.code does not have a binding to any value set. Yet, it is clear, throughout this IG, that CMS-HCC codes are the predominate example. This is further confirmed by inserting this example into the definition of the element that is meant to be used in other situations too. Do not insert examples into a definition of the element, instead bind HCC as an example binding for the element. Change the element definition to: 

      "Category codes that are used to indicate the risk strata assigned to this condition grouping."

       

      And then use an example binding to the value set of all HCC codes (if that is what you want.)

      Note that this lack of providing a binding, if done elsewhere in the IG, should be fixed in a similar fashion where found elsewhere.
