Reconsider the LOINC code used for Employment status to align with ODH

      Background:

      US Core has developed US Core Common SDOH Assessments ValueSet - commonly asked social questions as identified by FindHelp.org, a social service assistance tool. This is not intend to replicate the complexities and robustness of the FHIR Questionnaire and QuestionnaireResponse resources developed for SDOH assessment screening tools and other questionnaires. The Gravity Project is working on a more comprehensive set of codes to meet these challenges (this guide may leverage that work in the future).

      The codes supplied by FINDHelp.org were mapped to the most appropriate LOINC codes that were members of common screening protocols such as PRAPARE.

      The commenter is suggesting that the LOINC:

      67875-5|Employment status - current

      ( A Member of Protocol for Responding to and Assessing Patients' Assets, Risks, and Experiences [PRAPARE] panel )

      be replaced with the LOINC:

      74165-2| History of employment status NIOSH
      Term Description:
      Current and historical data related to an individual's employment status. Generally, employment status refers to whether or not a person has a job. In a healthcare setting, employment status may be used to determine appropriate probing questions for occupational exposures and occupational history. For example, someone who is unemployed or has chosen not to work may be prompted to provide information about previous jobs. Employment status is not the same as compensation and sector employment type [LOINC:85104-8].

      Note that the DRAFT USCDI V3 adds the Patient Demographic Data Element:

      Occupation: A self-reported term for the type of work done by a person. For a military position, this is the primary occupational specialty.

      Which may appear in a future versions of US Core and alleviate the commenters concerns.

      Reasoning:

      We disagree that these LOINC concepts are equivalent, because they are neither defined the same nor used in the same context.

      Proposed Changes:

      There is no action needed by US Core on this issue.

      ODH uses the LOINC code 74165-2 for Employment Status. It would be frustrating to have this recorded twice using different LOINC codes. Suggest switching to the ODH profile and LOINC code for employment status.
