Consider adding MS on medicationrequest.requester choice type of Practitioner

      Given the intent, we will remove the Organization type from requester
      In accordance with current US Core Guidance (in both 4.1.0 and 5.0.0) for medication list requires patient be able to report a medication and to make a request for a medication (I.e., self-prescribed).  Based on this guidance, QI Core will maintain:

      • MedicationRequest.requester (Must Support) with reference to QI-CorePractitioner, QI-CorePatient, ADD QI-CorePractionerRole, REMOVE QI-CoreOrganization
      • Medicationrequest.reported.reportedReference (MustSupport) with reference to QI-CorePractitioner, QI-CorePatient, ADD QI-CoreRelatedPerson, ADD QI-CorePractionerRole, REMOVE QI-CoreOrganization
      • Add 3 examples with explanation:
        • Practitioner ordered medication (I.e., Intent = order)
        • Patient reported medication (reporting a MedicationRequest with requester is still a practitioner so the Intent = order)
        • Patient self-prescribed (reporting a MedicationRequest with requester is the patient - and that means the intent = plan)
