Consider updating choice type for Encounter.referenceReason to mirror US Core

      US Core references base FHIR references for resources that US Core does not profile. QI-Core, however, does profile some of these base FHIR Resources and the References should be to QI-Core profiles rather than the base FHIR Resources. Specific changes to apply:

      • QI-Core Encounter.participant.individual - currently references QI-CorePractitioner - to be consistent with US Core, add QI-CorePractitionerRole and QI-CoreRelatedPerson and only have Must Support for Practitioner.
      • ADD Must Support for QI-Core Encounter.reasonReference and reference QI-Core ConditionEncounterDiagnosis (new profile), ADD MS for QI-Core Condition, QI-Core ImmunizationRecommendation, QI-Core Procedure, QI-Core Observation.
        ALSO:
      1. Add a ConditionEncounterDiagnosis profile based on US Core's new ConditionEncounterDiagnosis profile
      2. Retain Must Support for Encounter.diagnosis and assure that Encounter.diagnosis.condition can Reference QI-CoreCondition, QI-CoreConditionEncounterDiagnosis, and add QI-CoreProcedure as well to avoid the need to have a procedure extension. This is consistent with base FHIR R4 and proposed R5 definitions forEncounter.diagnosis.condition as "The diagnosis or procedure relevant to the encounter" - in BOTH R4 and R5.
