      As discussed in the 2021-11-04 CPG/CQI VSD Value Set profile call, https://confluence.hl7.org/pages/viewpage.action?pageId=130485487 

      and from information Rob_McClure uncovered as to the need for these extensions (added as a result of the VSDP project), they are not necessary.

      Remove the two extensions from the specification.
