DEQM Gaps in Care Bundle slicing is inconsistent

      according to the following:

      1. keep the first slice of composition
      2. remove second slice of measure report
      3. add narrative conformance requirement to include all references to patient specific resources including evaluated resources referenced by the included measure report and including referenced guidance response resources
      4. add discussion about the structure of the gaps in care report ideally including a picture that illustrates the complete structure of the gaps in care bundle and requirements for document type bundles

      Should the DEQM Gaps in Care Bundle specify an DetectedIssue entry slice?

      The slicing is:
      Composition 
      Measure

      Which are like the "header" and "body".

      Seems like it should be:
      Composition
      Measure
      DetectedIssue

      To be the "header", "body", and "conclusion".

      If not, then why keep Composition and Measure as slices?
