      Remove location from reporter element.

      Add a location element that can be a reference to a location or organization that represents the location sending the report.  Cardinality 0 to 1.

      Add the example of a public health indicator being sent from multiple locations.

    Description

      The MeasureReport resource has first-class elements corresponding to the Who, What, and When of the data covered in the report, but does not have a corresponding element for the "Where". This presents difficulties when multiple organizations are reporting from the same facility, meaning that we have to work around the issue by considering facility as a supplemental data element. We are proposing an extension for location and would like to consider that extension as a bridge toward the functionality in MeasureReport itself.
