Description
The hierarchy here doesn't seem to be complete. For example, home health, pre-admission and virtual would presumably all be sub-types of outpatient? If not, then the definitions need to be refined. If we're going to have an extensible value set, the relationships and boundaries between the concepts need to be extremely clear so that when implementers map, there's no confusion about what maps to what.
