Add Preferred Method of Contact Extension to PractitionerRole and Patient

      There is an existing extension iso21090-preferred that can be added on ContactDetail, Address, or the new (not yet applied as of Jan 2022) ExtendedContactDetail data types.

      If that doesn't work, a v2 specific extension could be used.  The FHIR extension lets you be more specific than v2, since in v2 you can have multiple phone numbers, and if you say that phone is your preferred contact type, that is ambiguous.  We don't want to carry the ambiguity forward into FHIR, so if preserving that ambiguity is necessary, use a v2 specific extension.

    Description

      As part of the v2-to-FHIR project we are mapping commonly used fields in key v2 messages to FHIR resources so that messages can be transformed into bundle of FHIR resources. PRT-23 is the Preferred Method of Contact field but there is no cognate existing data element in FHIR. We suggest that a standard extension to PractitionerRole and RelatedPerson (and maybe Practitioner) be created to allow exchange of this concept. 

       

      We suggest a CodeableConcept value using value set http://terminology.hl7.org/CodeSystem/v2-0185 with a binding of preferred (as table 0185 is HL7 defined).

       

      The v2 description can be reused as part of the extension narrative:

      This extension contains the preferred method to use when communicating with the contact.
