Add MedicationDispense.whenPrepared
And add guidance to indicate this element is to be used in cumulative medication duration and its use must be considered for each measure on a use case basis.
Will also add MedicationDispense.whenPrepared as a coalesce to the examples using MedicationDispense for cumulative medication duration; and add guidance regarding when to use it when available.

Description

Add MedicationDispensed.whenPrepared to the profile as MUST SUPPORT. In practice, .whenHandedOver is not always populated, but .whenPrepared usually is populated. eCQM logic may want to use .whenPrepared if .whenHandedOver is null.
