Given the complexity of the use cases and personas and the realities of an actual encounter, it is hard to see how the provider would be conducting the bulk of this goal setting work, but rather, that SWS would be consulted and would follow for the provider. This role should be added and enhanced throughout the entire ballot write up since this is the specialty with the most appropriate education and experience to address the concerns in the use cases (housing, food, transportation).

The provider and patient establish goals and agree on the specific referrals/interventions/exchanges (not depicted in this diagram) and the patient provides consent to share information with the relevant organizations.
