The intro to this section seems to imply that the only way you can have an SDOH Condition is if you first have someone fill out a scoring instrument\. I think that's pretty unlikely\. It's possible that some payers will mandate that a scoring instrument be used to allow reimbursement, but I expect there'll be exceptions for certain conditions/circumstances\. If a patient says "my house burned down", the clincian isn't going to say "here, fill out this form so we can see if you qualify for housing supports"\.\.\.
