There is a need to identify the observation as the result of an SDOH survey instrument. We be able to identify Social-History observations and to differentiate those what are the result of a survey and those that are based on a different source (e.g. an interaction between a provider and a patient or from the result of an intervention) Will provide explanation (guidance) for the use of these required category codes for the above purposes.
