Add the overall internal flow for each of the actors that describe the creation and updating of the various artifacts that have been or will be exchanged (questionnaire to observations and unverified condition (health concern), interaction with provider/patient, setting to verified and/or promoted to the problem list, goal generation (where appropriate), creation of intervention(s)/referral(s), recording of actions taken, updating goals and conditions)

This is the provider view of the world – other actors will have similar but not identical view (workflow)

      Description

There are muliple steps here. First the survey is completed. Then, Observations - including risk scores are generated from the completed survey. Finally a GP leverages those scores together with information gleaned in talking with the patient to record Conditions and add them as concerns to the patient's problem list. Conditions are NOT auto-generated from the survey results.

      Existing Wording:

The patient interacts with the provider and takes a risk survey that creates the specific health concerns defined by the survey and the patient’s responses.
