Required support for dataAbsentReason extension for patient first name

      There is a check box on the mother's worksheets for "name not chosen". It would be ideal to have the data absent reason on the name element rather than the given/family levels, but US Core has the following invariant:

      "us-core-8: Patient.name.given or Patient.name.family or both SHALL be present"

      It would be possible to fill in the family name but not have chosen a first name. Will add guidance to use the code "temp-unknown" when "name not chosen" is checked.

    Description

      For both the Patient - Child and Patient - Decedent Fetus profiles, support for the data-absent-reason extension is require for the patient's given name. This element has a cardinality of 0..*, so isn't required if it's not known. What is the rationale for providing a data absent reason? Many of the options in the value set (masked, not applicable, unsupported, etc) don't seem relevant to a vital records workflow.
