Description
1) Include a definition before providing the example. ( e.g.: "Incomplete Healthcare Information (IHI?) refers to a circucumstance in which [the healthcare information documented is not sufficiently comprehensive for the purpose of accurate quality measurement analytics]. 2) Also recommend using a different example. Since in this situation the patient would be getting the needed treatment, it is not a prime example of a situation related to a care gap - this example more closely describes a utilization management situation.

Incomplete Healthcare Information: For example, a request for cancer treatment without providing date of diagnosis or stage of illness at the time of diagnosis to support effective care coordination.
