Please provide specification for app context for a "generic" SMART app (e.g. for DTR) that includes (at a minimum)

1) the context communicated to the CDS endpoint

2) the URL for the "payer" endpoint service that provides the FHIR Questionnaire and CLQ

3) the JWT necessary for the app to access the payer endpoint
