_Admission, Discharge, and Transfer Event Notifications: CMS is modifying Conditions of Participation (CoPs) to require hospitals, including psychiatric hospitals and CAHs, to send electronic patient event notifications of a patient’s admission, discharge, and/or transfer to another healthcare facility or to another community provider or practitioner. This will improve care coordination by allowing a receiving provider, facility, or practitioner to reach out to the patient and deliver appropriate follow-up care in a timely manner. This policy will be applicable 6 months after publication of this rule.

_(4) To the extent permissible under applicable federal and state law and regulations, and
not inconsistent with the patient’s expressed privacy preferences, the system sends notifications
directly, or through an intermediary that facilitates exchange of health information, either
immediately prior to, or at the time of:
The patient’s discharge or transfer from the CAH’s emergency department (if
applicable).
(ii) The patient’s discharge or transfer from the CAH’s inpatient services (if applicable)._

"
Transfer Notification should only be used if
your organization calls it a transfer when the patient moves from the emergency room to an inpatient status OR
your organizations calls it a transfer when a patient moves from inpatient to another facility like an inpatient rehab facility.
You should not use Transfer notification for bed changes within the same facility, i.e. ICU to a stepdown
If your facility only uses Admit and Discharge transactions, for the above scenarios, you will not use Transfer Notifications."
