      We require focus.reference attribute so there is no question how to find the resource inside the bundle. ( e.g., use of only identifiers which is not desired behavior)

      The element MessageHeader.focus.reference is a property of Reference. Typically if a reference is required, it is only required at the top level, and constraints are not typically defined for the properties within Reference. This is reinforced by the fact that the Reference datatype has a constraint that indicates "At least one of reference, identifier and display SHALL be present (unless an extension is provided)."
