FHIR-25617 Update VTE example to use QI-Core 2019 version

Description

Links to CQFM Library and CQFM Measure point to FHIR STU3 versions. Shouldn't they point to the R4 based version of QM IG? (just like the latest QI-Core is referenced below)

BTW, this comment also applies to other sections of the IG, such as 2.3.4 and 2.4.3 (both listing the CQFM Measure profile)
