Consider adding biospy to CancerDiseaseStatusEvidenceTypeVS - MCODE #139

      The review is of course correct that biopsy procedures are commonly done to diagnose cancer. When a biopsy is done, the nature of the cancer is revealed through pathology. The intent is for SCT#252416005 "Histopathology test (procedure)" to be used when there is a biopsy that contributes evidence of the cancer disease status.

      We will add documentation to this effect to make it clearer to the user of mCODE.
