Encounter.period needs to allow flagging admission/discharge dates as "planned"

      Introduce a new extension for the plannedStartDate (datetime) 0..1 which corresponds with the appointment.startDate and also the encounter.period.start. (Comments will include text that this covers the Planned Admission date too) which applies at the top of the Encounter

      Introduce a new extension for the plannedDischargeDate (datetime) 0..1 which applies at the top of the Encounter

      These extensions will be included to be trialed in R4, and if this is widely supported, promote to core properties for R5
