Expand the contexts of use for the ordinal value to include the ValueSet expansion

"Use on Element ID Coding, Element ID Questionnaire.item.answerOption, Element ID CodeSystem.concept or Element ID ValueSet.compose.include.concept"

Also include a technique for the ValueSet $expand operation to return extensions, and optionally properties.
