      There's nothing in the IG-publisher or in the official templates that would introduce extensions like this.  If the IG is using the current tool stack, we're quite confident the problem won't occur.  If it is still occurring, please re-submit and point to a CI build file where the problem can be seen.

      We do see in source/input/TS.xml a reference this:

      <element id="TS.value">
        <extension url="http://www.healthintersections.com.au/fhir/StructureDefinition/elementdefinition-dateformat">
        <valueString value="v3"/>
        </extension>

      That's not a tooling issue.  If you don't like it, you can (and should) remove

    Description

      The fhir-cda project (https://github.com/hl7/fhir-cda) contains 7 locations where a healthintersections extension is used. I would expect a FHIR core extension instead. From the qa.html:

      The extension http://www.healthintersections.com.au/fhir/StructureDefinition/elementdefinition-dateformat is unknown, and not allowed here

      The thing with this error is that you get this after building the |IG. This extension does not live in the source as far as I could find, so I'm guessing it is added during publication.
