Add balance back to account

We would like to see the balance added back to the account resource. We feel requiring multiple calls to account to retrieve the balance is burdensome to API consumers. The primary reason to retrieve the account resource is to view the balance. The FHIR server should be allowed to respond with the balance information if it deems it appropriate.
