The Encounter.statusHistory.code has a required binding vocabulary that is not inclusive enough of events. For example in IHE we are profiling the EMS encounter prior to delivery of the patient to the hospital. There are may events that happen during this that needs to be recorded as statusHistory. (e.g. EMS dispatced, EMS arrival at scene, etc)

Cloned from 16237, as that was improperly included in ballot against FHIR Infrastrure and must be closed as not-related
