      Comment:

      Finally, it remains puzzling that the reaction is not an associated Observation, possibly with an associated exposure observation. I gather that many systems record the reaction and infer a condition on a single interaction, so the close association is understandable, but I'd want the observation of a reaction to have the same query & extension support other observations have.
