Add an extension for primary diagnosis in the context of EpisodeOfCare

      (Refer to tracker 10544 which has a similar issue on Encounter that was modified from an extension to a core element)

      The episodeofcare resource will be update to include the following new backbone element diagnosis replacing the condition property.

      EpisodeOfCare.diagnosis 0..* backbone element

      EpisodeOfCare.diagnosis.condition 1..1 reference(condition)

      EpisodeOfCare.diagnosis.role 0..1 codeableconcept

      (referencing the new valueset from Encounter see tracker _10544)_

      EpisodeOfCare.diagnosis.rank 0..1 positiveInt (this is to be sequential per role type)

      And search fields will be udpated to support this new structure, specifically the condition reference, and also include the diagnosis role (to permit searching for all chief complaints)

    Description

      In our palliative care management application, we are working with patients in the context of episode of care, and we need to capture their diagnoses and distinguish the primary (admitting) one in this context.

      Encounter has an extension to allow indication of the "primary" diagnosis - http://hl7-fhir.github.io/extension-encounter-primarydiagnosis.html. Please add similar extension for primary diagnosis in the context of EpisodeOfCare.
