As we're modeling CDA in SD, it's clear to see that all CDA classes are based off of InfrastructureRoot (contain realmCode, typeId, and templateId) - and most of the SDTC extensions also include these.

However, every CDA element is based off of type "ANY" which includes @nullFlavor. None of the SDTC extension classes apart from Precondition1 and AllFalse include this attribute.

Proposal - add the attribute back in the CDA SDTC schema, and going forward in SD, keep these based on InfrastructureRoot/ANY like all the rest of the CDA elements.
