Description
In CDA-20014, the Problem Observation (V3) template in C-CDA had inline guidance related to SNOMED qualifiers above the value/qualifier conformance statement (CONF:4515-31870) updated. The same inline guidance update should be propagated to Problem Observation (V4) in the Companion Guide. The revised inline guidance as published in the C-CDA 2.1 Sep 2022 errata release is as follows:
The observation/value and all the qualifiers together (often referred to as a post-coordinated expression) make up one concept. Qualifiers constrain, or provide greater specificity to, the meaning of the primary code, and cannot negate it or change its meaning. Qualifiers can only be used according to well-defined rules of post-coordination and only if the underlying code system defines the use of such qualifiers or if there is a third code system that specifies how other code systems may be combined.
For example, in cases where SNOMED CT does not have a precoordinated code that would be appropriate for the problem list, concept post coordination may be used in CDA following the principles outlined in HL7 Version 3 Implementation Guide: TermInfo - Using SNOMED CT in CDA R2 Models, Release 1 using the V3 CD Data type 1 style. This is shown in the Problem Observation (V3) Post-coordinated Problem Example. This example represents a family history condition that was also elevated to the problem list to avoid missing pertinent data that may or may not be present in the Family History Section.
