As discussed in this pull request for the C-CDA examples repo, several of the XML examples in the C-CDA IG contain invalid XML and should be tidied up.

This ticket exists to track applying the changes made to those examples in Trifolia as well. The updated versions of the files can be viewed from that GitHub commit.
