    Description

The Pharmacist Consult Note template claims that the C-CDA ClinicalDocument column in the template requirements table represents:

The XML element within the ClinicalDocument where the described information is located.
“---” indicates that the concept does not pertain to the C-CDA structure. Either the concept doesn’t apply or the concept
is incorporated differently than how it is incorporated in C-CDA
“ClinicalDocument” is the XML root element of a Pharmacist Consultation Note.

However, it then contains values such as "ID", "Address," "Telecommunication," "Name," and "Gender", none of which are elements in the C-CDA document (instead they'd be id, addr, telecom, name, and administrativeGenderCode).

IMO, this is a good argument for using standard tooling for these templates (ART-DECOR or, better yet, Trifolia); this would become a lot more standard and easier to maintain.
