The serviceEvent/performer in the Patient Generated Document Header has a different binding on the assignedEntity/code than the US Realm Header. Therefore it's necessary to keep the performer (and therefore its functionCode) in the Patient Generated Document Header template.

Since we're keeping the functionCode, we'll update the binding to the same value set as in ~~CDA-1767~~ (2.16.840.1.113883.1.11.10267)
