Within the Progress Note template, the ClinicalDocument/documentationOf branch is encouraged with a SHOULD, but the following primitive constraint at the documentationOf/serviceEvent/effectiveTime level generates errors even when there is no documentationOf branch:

If a width element is not present, the serviceEvent SHALL include effectiveTime/high (CONF:1198-10066).

Here is the applicable schematron from Trifolia:

We probably need to modify this to check to see if there exists a documentationOf/serviceEvent/effectiveTime node first, because it is only required with a SHOULD.
